Disease Management and Treatment of Acute and Chronic Illnesses:
- Heart disease and hypertension
- Infections: Sinus, cold/respiratory, urinary
- Primary gynecology (pap tests) and contraceptive counseling
Psychiatry and Behavioral Health Services:
For established patients of the health center, Chestnut Family Health Center also offers psychiatry and behavioral health services. Health center patients will be referred to these specialty service providers by their Chestnut primary care provider when appropriate.
Basic onsite laboratory services are available at the time of visit or by appointment.
Payment and Sliding Fee Schedule:
Chestnut Family Health Center is a community-based health center. We accept all forms of Medicaid. In addition, we accept: Medicare, Blue Cross Blue Shield, Health Alliance, and Country Healthcare. Please bring your current medical card with you for each visit. We will bill Medicaid directly.
Access to a sliding fee schedule for patients who qualify based on family size and income is available. Chestnut's financial intake staff are happy to meet with you to review eligibility requirements and establish a sliding fee rate.

The U.S. Department of Health and Human Services recently recognized Chestnut Family Health Center as a “Health Center Quality Leader”, an “Access Enhancer”, "Clinical Quality Improvers", and a “Health Equity Advancer” for providing the highest quality care even as we led in expanding services to an ever-increasing number of patients.
This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
Chestnut Family Health Center has achieved accreditation and Primary Care Medical Home (PCMH) certification from The Joint Commission – only 10% of all ambulatory health centers in the U.S. qualify.
