Health care is available for children and adults. We treat your immediate concerns like colds, the flu and infections and ongoing conditions like high blood pressure and diabetes.

  • DOT and other physicals
  • Women's health
  • Pregnancy care and family planning services
  • Weight management
  • Treatment for behavioral health and wellness issues, such as depression and anxiety
  • Same-day sick visits

Payment and Sliding Fee Schedule

Chestnut Family Health Center is a community-based health center. We accept all forms of Medicaid. In addition, we accept Medicare, Blue Cross Blue Shield, Health Alliance, and Country Healthcare. Please bring your current medical card with you for each visit. We will bill Medicaid directly.

Access to a sliding fee schedule for patients who qualify based on family size and income is available. Chestnut's staff members are happy to meet with you to review eligibility requirements and establish a sliding fee rate.
