All populations in Illinois can receive the vaccine regardless of insurance coverage or immigration status, including individuals who are undocumented. There is no co-pay for individuals with insurance and no charge for people who are uninsured

Anyone over age 18 is eligible to get the COVID-19 vaccine at Chestnut Family Health Center. There is NO CHARGE to get the vaccine. 

If you would like to make a vaccination appointment at either Chestnut Family Health Center location, please follow these steps:

• If able, please print and complete the inactive vaccine consent and administration record form: Inactive Vaccine Consent and Administration Record Form If you cannot print and fill this form out before your appointment, the form will be available at the time of your appointment.
