Our Providers

Dr. Rogers is a graduate of Western Illinois University and Southern Illinois School of Medicine. He has more than 40 years of family and emergency medicine experience including providing high-quality health care to children, adolescents, and adults.

As medical director, Dr. Rogers’ goal is to “help guide Chestnut Family Health Center in a direction so the community will see it as a valuable and accessible resource for primary health care, behavioral health care, and dental care.”

He is Board Certified by the American Board of Family Medicine and is a Certified Physician Executive.

Dr. Rogers is interested in seeing a wide range of patients who have different needs. He has a special interest in skin conditions and minor office procedures.

When he’s not practicing medicine, he enjoys horticulture, reading, and spending time with his family. He is also an avid runner and has completed the St. Jude Memphis to Peoria Run twelve times.

Dr. Brockhouse is a family medicine physician who has practiced in Bloomington- Normal since 1991. She earned both a Bachelor of Arts and Master of Science from Illinois State University. She is a graduate of Chicago College of Osteopathic Medicine, and completed her residency training at SIU.

Dr. Brockhouse is a board-certified physician focusing on Medication Assisted Recovery. She has been treating Substance Use Disorder since 2014. Dr. Brockhouse’s passion is to provide evidence-based, patient-centered care to individuals suffering from Substance Use Disorder.

Dr. Brown was born in Colorado but grew up in Nova Scotia, Canada. He holds undergraduate degrees in liberal arts and biology. His medical degree and Master of Public Health is from St. George's University School of Medicine. In his spare time, he enjoys traveling and trail running. Dr. Brown is a part of the OSF Family Medicine Residency program.

Melinda is a Board Certified Family Nurse Practitioner. She has her master’s degree in Nursing from Olivet Nazarene University and her bachelor’s in Nursing from Indiana Wesleyan University. For more than eight years, Melinda was an emergency room nurse, which gives her extensive knowledge about caring for newborns to geriatrics. She is excited to engage in patient relationships to help improve overall health and wellness. Melinda is always available, takes pride in caring for her patients, and strives to live the Chestnut mission statement: Making a Difference: Improving quality of life through excellence in service. She is committed to go above and beyond to make sure patients feel this in their care. She looks forward to meeting you!

Kelly is a board-certified Family Nurse Practitioner with over 20 years of nurse practitioner experience. She completed her Master’s in Public Health from the University of Illinois in Chicago. During her career in Central Illinois, Kelly spent many years at Advocate Bromenn Medical Center in various roles and eight years at Planned Parenthood.

Dr. Gupta was born and raised in an agricultural village in Nepal. He is a graduate of Kathmandu University School of Medical Sciences, and his goal is to become a competent and compassionate physician. Dr. Gupta believes in lifelong learning and looks forward to putting his residency training into practice to serve the community. Dr. Gupta is a part of the OSF Family Medicine Residency program.

Dr. Hanson is from LaSalle-Peru, Illinois. She is a graduate of Illinois Wesleyan University and Loyola University Stritch School of Medicine. She lives in Bloomington with her husband, Mark, and her Siamese cat, Simon. Dr. Hanson is a part of the OSF Family Medicine Residency program.

Dr. Hurlock grew up in Arizona. He attended Arizona State University for his undergraduate degree and Midwestern University for medical school. Dr. Hurlock is a part of the OSF Family Medicine Residency program.

Dr. Kettering grew up in Northeast Ohio and earned his medical degree from Des Moines University. He is board certified in Family Medicine. Dr. Kettering loves seeing patients of all ages at different stages of life. Dr. Kettering has four kids and enjoys tennis, golf, and classic cars.

Cindy has a long history in nursing – working primarily in cardiac care and occupational health. After receiving a master’s degree in nursing from the University of Illinois and a doctorate degree in education from Illinois State University, Cindy taught nursing for several years at the baccalaureate and master’s levels at Saint Francis Medical Center College of Nursing and Methodist College of Nursing. While a very rewarding career path, Cindy missed having her own patients and returned to school at Mennonite College of Nursing at ISU for a post-master’s certificate as a Family Nurse Practitioner. Later, she was drawn to care for those struggling with mental health concerns, and again returned to ISU and obtained a second post-master’s certificate as a Psychiatric Mental Health Nurse Practitioner. Cindy enjoys teaching and collaborating with patients to create an individualized treatment plan that best meets their needs. As a proud member of the Chestnut Family Health Center care team, Cindy strives to make a positive difference in the lives of others and to help each patient achieve and maintain a happy, healthy life.

Brittany is a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP) through the American Nurses Credentialing Center. She received her post-graduate certification and Masters in Adult Gerontology (AGNP) from Maryville University. Brittany has worked in many different practice areas as a nurse and nurse practitioner but has always had a passion for mental health. Brittany’s ambition is to improve access to mental health care and to reduce the stigma with an overall goal of educating the public. Brittany believes in a whole-body approach and feels mental health, just like physical health, requires treatment when changes occur.

Dr. Rizvic attended the University of Utah where he earned a bachelor’s degree in science and chemistry. He also attended Jagiellonian University Medical College in Krakow, Poland. He loves to travel and more importantly, he loves to volunteer. Dr. Rizvic is a part of the OSF Family Medicine Residency program.

Dr. Yang was born and raised in Memphis, TN. His parents were refugees to the United States, and he is thankful to be part of a first generation born in the U.S. He graduated from the University of Memphis with a bachelor’s in Exercise and Sports Science. He went on to attend medical school at Saba University. In his spare time, he enjoys exercising, sports, and trying new restaurants. Dr. Yang is a part of the OSF Family Medicine Residency program.

Dr. Garner has practiced internal medicine in Illinois since 1988. Dr. Garner received his bachelor’s degree Summa Cum Laude from University of Illinois at Urbana Champaign. He received his MD degree from the University of Illinois at Urbana-Champaign. He completed his internship and residency at Saint John’s Mercy Medical Center in Saint Louis, MO. Dr. Garner is board certified in general internal medicine and in Hospice and Palliative Care Medicine and in Addiction Medicine. He is a fellow of the American College of Physicians.

Paige is a board-certified family nurse practitioner. She received her Master of Science in Nursing from the University of Alabama at Birmingham and her Bachelor of Science in Nursing from Southeast Missouri State University. She is certified through and a member of the American Academy of Nurse Practitioners. Paige aspires to promote overall wellbeing and improved quality of life to patients of all ages.

Dr. Habib is a licensed physician and surgeon with a board certification in psychiatry. Dr. Habib received his medical degree from Dow University of Health Sciences and his board certification through the American Board of Psychiatry and Neurology, Inc. Dr. Habib has been practicing for more than 20 years with a focus on adult psychiatry. In addition to working at Chestnut Health Systems, Dr. Habib has a private practice and admitting privileges at both Gateway Regional Medical Center and Mercy Hospital.

Jaime is a Psychiatric Mental Health Nurse Practitioner licensed in Missouri and Illinois who joined Chestnut Health Systems in 2021. She attended the University of Missouri – Saint Louis to receive her Doctor of Nursing Practice degree. Prior to this, she worked for nineteen years in an array of nursing areas including critical care, medical-surgical, behavioral health as well as teaching undergraduate clinicals.

Jenia is a board-certified Family Nurse Practitioner. Jenia completed her masters in Nursing Science at Maryville University and her Bachelors in Nursing Science at Southern Illinois. She is a member of the American Association of Nurse Practitioners.

Emily is experienced in pediatric rehabilitation and primary care as well as adult acute care and medicine. She started her journey at Chestnut Health Systems as intern for two years before her current position. Emily earned her Doctorate in Nursing from Southern Illinois University Edwardsville and her Bachelors of Nursing from Goldfarb School of Nursing at Barnes Jewish College in St. Louis, Missouri. While pursuing her doctorate, she taught nursing students. Emily is native to the Southern Illinois region and enjoys serving the people of her community.

Ellen is a licensed professional counselor and certified personal trainer with the state of Illinois. She has worked with Chestnut since 2015. She oversees the wellness groups in Belleville and the InShape Program.

Veronica is Chestnut Health Systems’ lead Advanced Practice Registered Nurse (APRN). She has been a nurse practitioner with Chestnut since 2014. She completed her Doctor of Nursing Practice at Southern Illinois University Edwardsville and a Master’s in Nursing Science, Psychiatric and Mental Health at the University of North Dakota. She earned Bachelor of Science degrees in both Nursing and in Community Health. She has worked with individuals of all ages and in various settings including inpatient and outpatient mental health, urgent care, addiction medicine, and adolescent residential treatment. Her goal is to provide holistic care and to work together to improve client health and wellness.

Molly has been with Chestnut Health Systems since 2018 and joined the Chestnut Family Health Center in 2021. She is a licensed clinical social worker and earned her Master’s Degree in Social Work from Southern Illinois University Edwardsville, specializing in micro social work. Molly enjoys working with a variety of clients by meeting them where they are to provide individualized support through therapy.

Laurie is a Family Nurse Practitioner (FNP-C), board certified by the American Academy of Family Nurse Practitioners, and has her Doctor of Nursing Practice (DNP) from Saint Louis University. 

Laurie is a lifelong learner and is currently working on obtaining her Psychiatric Mental Health Certificate through the University of Missouri, Columbia. 

Laurie has a variety of experiences as a nurse including assisting in opening a school-based health clinic in rural Missouri which provides primary care to pediatrics and adults.

Laurie's goal is to help clients reach their healthcare goals and improve access to quality healthcare, including mental health and medication-assisted recovery for all segments of the population.

Nick is a board certified Nurse Practitioner with a drive to improve access, education and primary care to underserved populations. He earned a Master’s degree from Vanderbilt University. Nick is board certified in Adult-Gerontologic Primary Care. He has worked with many different cultures and populations across the country and looks forward to serving the people of this area.

Robyn has been with Chestnut Health Systems since 2014. She is a board-certified Family Nurse Practitioner (FNP) and Psychiatric Mental Health Nurse Practitioner (PMHNP). She specializes in child and adolescent mental health. Robyn enjoys working with clients in numerous settings including school, residential treatment, and outpatient. Robyn completed her Masters in Nursing Science at Southern Illinois University-Edwardsville and her post-master’s for mental health at the University of Missouri-St. Louis. She is certified through the American Nurses Credentialing Center. She is also an adjunct faculty member at Southern Illinois University-Edwardsville and at Maryville University. Robyn’s goal is to provide her clients with the tools to live a successful life.
