We are partnering with Carle Health and the McLean County Health Department to offer school physicals to McLean County students entering kindergarten, 6th grade, 9th grade, and new students joining the district!

Immunizations will be provided as part of the exam. Parent/guardian must be present and provide identification at the time of appointment.

What to Bring to Your Appointment:

  • Parent/guardian I.D.
  • Immunization records
  • Insurance card(s)

Insurance Accepted:

  • Medicaid and private insurance
  • Uninsured patients welcome
  • Discounted fees available
