As part of Chestnut Health Systems, you’ll also have easy access to mental health and psychiatry services, primary care, lab tests, a pharmacy, community resources, and more. We’re here to support your health so you can live fully!

Chestnut is a Patient-Centered Medical Home, which means a team of doctors, nurses, and other health experts will work together to take care of you.

You are the most important part of the team! It’s important to tell us about your health and any concerns you have. This includes changes in how you feel, the medicines you take, any trips to the emergency room or hospital, visits with other doctors outside of Chestnut, and medical tests done elsewhere.

HOW WE CAN HELP

At Chestnut Family Health Center, we care for both your physical and behavioral health.

What is behavioral health?
Behavioral health is how your daily habits affect both your body and mind. It includes things like eating and drinking habits, exercise, substance use, and mental health challenges.

You can visit us for common illnesses like colds, flu, fevers, sprains, and strains. We also help manage long-term conditions like diabetes, high blood pressure, and heart disease.

You will choose a doctor or nurse to be your main provider. This person will work with other staff members at Chestnut Family Health Center and, when needed, specialists outside of Chestnut to give you the best care.

If your provider orders bloodwork or other labtests, you may be able to get them done at our office. You can also fill your prescriptions at our in-office pharmacy.

The Joint Commission has accredited Chestnut Family Health Center as an ambulatory care provider and a certified Primary Care Medical Home.

The U.S. Department of Health and Human Services has recognized Chestnut Family Health Center for providing the highest quality care and for expanding services to more patients.
