Community Care Coordinators will screen, refer to services, and coordinate treatment services for participants in two detention centers. This project will: a) implement standardized screening (using a modified versions of the GAIN-Q4 instrument) to measure the risk of recidivism and need for behavioral health (substance use and mental health) treatment and identify who to refer/divert to behavioral health treatment or other services; b) refer detainees to behavioral health, substance use treatment and primary care providers in the community who are able to provide services virtually based on screening results; c) work with providers to deliver services virtually while the participant is in detention and will follow-up with the participant and coordinate care with those providers for 12 months post release; and d) will facilitate the R1 Learning ® curriculum in group settings or individually with detainees while in detention and via Zoom after release.
