Primary Care for All Ages We believe in prevention. We live out the philosophy by emphasizing wellness exams, immunizations, and disease screenings. We can also help manage your chronic conditions, such as high blood pressure or diabetes. |

Behavioral Health Mental and physical health go hand in hand. If you struggle with substance use, we can provide you with medication that eases withdrawal symptoms to help you quit safely. You can also receive counseling and mental health medication management (psychiatry). |

Case Management / Social Work You’re not alone. Our care coordinators and case managers will help connect you to community resources and benefits, such as insurance enrollment. |

Lab and Pharmacy We offer lab and pharmacy services, including blood draws, for established patients. You may order and pick up prescriptions at the Genoa Pharmacy located in the building. |

We Accept Medicaid, Medicare, Private Insurance Chestnut provides quality services while maintaining affordable fees. We will help you determine fees, check your insurance benefits, and apply for any available payment assistance. We offer a discounted fee to all patients based on eligibility. Chestnut accepts private insurance, Illinois and Missouri Medicaid/Medicare, and cash, check, or credit card. Please bring your medical card or insurance card to each visit. We will bill your insurance company. |
