Sliding Fee Scale

All Chestnut Health Systems primary care and behavioral health patients have access to a sliding fee scale (also available in Spanish). Your family size and income determine whether your care will be billed based on the scale. When you become a patient, our staff will help you to learn whether you qualify. No patient will be denied services based solely on the patient’s inability to pay. You will need to provide the following:

  • Identification: Acceptable forms include a State driver’s license or identification card, passport, or similar identification from other countries of origin.
  • Family size: Includes dependent children, spouse/partner, children up to age 26 who are attending college*, and any individual for whom the applicant has legal responsibility and who is shown as a dependent on their income tax filing.
  • Income: Please present one month’s paycheck stubs for each family member working and living with you.

We will re-evaluate this information every 12 months. If your family size or income changes at any time, please let us know.
