Chestnut Family Health Center provides quality health care to people who live at or below 200 percent of federal poverty guidelines in McLean and Madison counties in Illinois.
Our Approach to Care
Chestnut Family Health Centers are patient-centered medical homes. That means we provide health care that is
- Based on Relationships. We partner with patients. We recognize and respect their unique needs. We support patients as they learn to manage and organize their own care at the level they choose.
- Comprehensive. We are able to meet the large majority of each patient’s physical and mental health care needs. That includes prevention and wellness, caring for serious illnesses, and caring for ongoing illnesses.
- Coordinated. We help with transitions among different types of care (the health center, hospitals, home health care, and community services and supports). We excel at building clear and open communication among patients and families, the health center, and members of the care team.
- Accessible. We offer shorter waiting times for urgent needs as well as around-the-clock telephone access to members of the care team.
- Safe and high quality. We offer treatment that is based on science. We involve patients and their families in the care planning process. We measure how well we are doing and are constantly looking to improve.

The Center is a program of Chestnut Health Systems, which has been providing services in our community since the early 1970s. All of our physicians, physician assistants, and advanced practice nurses are dedicated to providing medical care which is specifically tailored to your needs and the needs of your family including primary health care for all ages, psychiatry, and behavioral health.
Primary Health Care for all Ages
We strongly believe in preventative medicine and encourage annual physical exams, gynecological care, immunization updates, and cancer screenings. We are always happy to discuss lifestyle issues that may improve your health. Your care is performed and managed by a medical professional in collaboration with other health professionals, and utilizing consultation or referrals as appropriate. We strive to promote effective communication with patients and encourage the role of the patient as a partner in health care.
Psychiatry and Behavioral Health Services
For established patients of the health center, Chestnut Family Health Center also offers psychiatry and behavioral health services. Health center patients will be referred to these specialty service providers by their Chestnut primary care provider when appropriate.
Payment and Discounted Fees
We currently accept Illinois Medicaid, Illinicare, Meridian Health Plan, and Molina. In addition to those Medicaid plans, we also accept Medicare, Blue Cross Blue Shield PPO, Health Alliance PPO, and Coventry Healthcare. Participation in these plans is subject to change, please contact us directly for verification. Please bring your current medical card and/or insurance card for each visit. We will bill your insurance directly.

Access to a sliding fee schedule (also available in Spanish) for patients who qualify based on family size and income is available. CFHC staff will assess your eligibility for the sliding fee discount at the time of intake and annually thereafter. If there are changes in your income and/or family size, please contact CFHC staff. Our staff would be happy to meet with you to reassess your eligibility for the sliding fee discount. |
